Inclusion / Deletion of Family member's name in Ration Card

When the Applicant's ( Consumer ) wanted to Add or Delete any of the Family member's name in Ration Card then he has to follow the following below process.
  1. Completely filled FORM D1-A will have to be submitted either in the office of District Food and Supplies Controller / Assistant Food and Supplies Officer / Inspector Food and Supplies OR In the concerned Tehsil / District Level e- Disha Centre between 9AM to 1PM in any of the working days.

  2. Applicant will submit two group photos of his family alongwith the Application form.

  3. Applicant will also submit his original Ration Card along with any one of the below documents :-
    a) Surrender Certificate ( In case of marriage of wife / daughter in law )
    b) Birth Certificate ( In case of newly born child )
    c) Death Certificate ( In case of death of the member of the family )

  4. The consumer will get a computer generated slip indicating the date as to when he should contact the concerned office for getting ration card.

  5. The ration card can be collected ( within 7 working days ) by the applicant from the office of District Food and Supplies Controller / Assistant Food and Supplies Officer /Inspector Food and Supplies OR concerned Tehsil / District Level e-Disha Centre where he submitted his application.

Fee Details :- Rs. 10 /- ( No Fee will be charged for BPL Families )
