Inclusion / Deletion of Family member's name in Ration Card
When the Applicant's ( Consumer ) wanted to Add or Delete any of the Family member's name in Ration Card then he has to follow the following below process.
Completely filled FORM D1-A will have to be submitted either in the office of District Food and Supplies Controller / Assistant Food and Supplies Officer / Inspector Food and Supplies OR In the concerned Tehsil / District Level e- Disha Centre between 9AM to 1PM in any of the working days.
Applicant will submit two group photos of his family alongwith the Application form.
- Applicant will also submit his original Ration Card along with any one of the below documents :-
a) Surrender Certificate ( In case of marriage of wife / daughter in law )
b) Birth Certificate ( In case of newly born child )
c) Death Certificate ( In case of death of the member of the family )
- The consumer will get a computer generated slip indicating the date as to when he should contact the concerned office for getting ration card.
- The ration card can be collected ( within 7 working days ) by the applicant from the office of District Food and Supplies Controller / Assistant Food and Supplies Officer /Inspector Food and Supplies OR concerned Tehsil / District Level e-Disha Centre where he submitted his application.
Fee Details :- Rs. 10 /- ( No Fee will be charged for BPL Families )
